Vaccination Policy An immunization record is required of all students. It must be completed and signed by the health care provider. We also require a tuberculosis screening questionnaire. If needed, a TB screen must be done. We highly recommend vaccination against meningococcal disease; a student who refuses must sign a declination statement and return the form to the Health Center. We recommend Gardasil, hepatitis A, and flu vaccines.
